How to Apply for Reimbursement

To secure reimbursement for the cost, most insurance companies will ask you for the following:

  • Invoice and Receipt. DNA Direct will email you a receipt after you order a test.
  • CPT Codes. These codes tell the company what lab work was done, and how much their policy will pay for it. CPT Codes for RPL Testing
  • Reimbursement Form. Each insurance company has their own form. Call your company to request this form. If you need help filling it out, you can call our Customer Service at the number and we will assist you.
  • Letter of Medical Necessity. Some insurance companies will ask for a letter from your doctor that confirms this testing is medically important. You can phone your doctor's office to request this. If your doctor has a question, they can call our Clinical Director at the number below.
  • ICD-9 Codes. These codes specify a medical diagnosis. Your doctor will need to provide these codes. If a letter of medical necessity is required, ICD-9 codes can be included in the letter.

Insurance Coverage for Pregnancy Loss

Insurance policies usually cover testing according to medical necessity, but coverage will depend on your individual policy.

Some Tests in a Panel May Be Covered. Because genetic testing for recurrent pregnancy loss (RPL) may include multiple tests, your insurer may pay for some of tests, but not others.

Check With Your Insurer. Insurance plans vary widely, and standard testing for RPL is often not covered because many insurers don't consider infertility an illness.

Getting Coverage. You are more likely to get coverage if testing is recommended by medical associations, as is the case with cystic fibrosis testing, which is recommended for couples planning a pregnancy. You are also more likely to get coverage if you can point to reasons for testing that are related to an illness that is covered by your insurer, such as blood clotting.
