How to Apply for Reimbursement

To secure reimbursement for the cost of the test(s), most insurance companies will ask you for the following:

  • Invoice and Receipt. DNA Direct will email you an order confirmation and receipt upon receiving your test order. Save these documents if you seek reimbursement.
  • CPT (Procedure) Codes. These codes tell the insurance company the specific procedures, performed in the lab, that are involved in the testing. The insurance company will check to see whether the codes are covered by your plan, and if so, whether they are fully or partially covered. CPT Codes for Ashkenazi Jewish Genetic Testing
  • ICD-9 (Diagnosis) Codes. These codes tell the insurance company the reason (s) the testing was done. They use this information to determine whether or not the tests were medically necessary. ICD9 Codes for Ashkenazi Jewish Genetic Testing.
  • Reimbursement Form. Each insurance company has its own reimbursement form. Call your insurer to request this form. If you need help filling it out, call our Customer Service number listed below and we will assist you.
  • Letter of Medical Necessity. Some insurance companies will ask for a letter from your healthcare provider that confirms that this testing is medically important. You can phone your provider's office to request this. If your healthcare provider has any questions, they can call our Clinical Director at the number below.

Insurance Coverage for Carrier Screening

Insurance companies usually cover Ashkenazi Jewish genetic testing according to their definition of medical necessity; coverage will ultimately depend on your individual plan.

Check with Your Insurer. Insurance plans vary widely. Ashkenazi Jewish genetic testing often is not covered by insurers, because individuals being tested are typically not seeking information that will affect their own health, but instead that of their offspring. Most insurance companies will pay for Ashkenazi Jewish genetic testing in a woman who is currently pregnant.

Some Tests in a Panel May Be Covered. Because Ashkenazi Jewish genetic testing includes multiple tests, your insurer may pay for some of tests, but not for others. You are more likely to obtain coverage if the testing is recommended by medical associations, as is the case with carrier screening for Tay-Sachs disease, Canavan disease, and Familial Dysautonomia, which is recommended by the American College of Obstetrics and Gynecology (ACOG) for individuals of Ashkenazi Jewish descent.

Getting Coverage. If insurance coverage is necessary for you, call your insurance company's customer service line before testing to obtain pre-authorization.
