Referring a Patient is Simple and Easy

1. Print and complete the DNA Direct Referral Form and Patient Instructions. Fax the completed form to 806-993-6708, Attention: Clinical Services. (Make sure you and your patient sign the form and check the appropriate box for medical records release authorization.)

2. DNA Direct will contact your patient to arrange a pre-test consultation and coordinate testing. A cheek-swab test kit and informed consent will be sent directly to your patient.
   
   
3. When testing is complete, DNA Direct will send you test results and a Physician Letter, complete with results interpretation, medical guidelines, and scientific references. Your patient will receive a Personalized Report.
